Neurogenic inflammation in asthma:
Antigen --> fatty cells --> neutrophil -->
Eosinophils -->Epithelial cell damage -----
Stimulation --> macrophage --> T-cell -->
Airway contriction
Hypersecretion
Vascular dilation
Plasmatic edema
Neurogenic inflammation --> airway remodeling -- repeat and repeat -- irreversible damage
Tx: early treatment with steroids
Tx: Pulmicol inhalation + prednisolone (Symbicort?)
1. diagnosis - pulmonary function test, f/u by PEF/PEV1
2. no cure -
3. chronic inflammation - life-long treatment
4. avoid risk factors
5. education (including family)
1. prednisolone 120# bring to home
2. effect: 4 hrs since taking drugs, peak effect at about 12 hrs
3. exacerbation: 8# st. as early as possible
4. double if exacerbing
5. monitor by PEF/PEV1
6. quit high dose prednisolone suddenly within one week
